2. मैं निःशक्त वर्ग की (निःशक्तता की श्रेणी) से हूँ। तथा जिसमें आने-जाने में काफी परेशानी होती है।
3. यह कि मेरे द्वारा सामाजिक न्याय एवं अधिकारिता विभाग को मुख्यमंत्री निःशुल्क स्कूटी योजना से
आवेदन पत्र में अंकित समस्त सूचनाएं मेरे ज्ञान एवं विश्वास के अनुसार पूर्ण रूप से सही एवं सत्य है।
मैंने कोई तथ्य नहीं छुपाया है।
4. यह कि मैंने पूर्व में मुख्यमंत्री निःशल्क स्कूटी योजना एवं अन्य किसी सरकारी एवं गैर सरकारी बैंक
/ संस्था की किसी भी योजना का कोई ऋण प्राप्त नहीं किया है।
5. यह है कि मैं ग्राम चिराना में इलेक्ट्रीकल की दुकान लगभग दो साल से चलाता हूँ तथा मेरे घर से
मेरी दुकान की दूरी लगभग 3 किलोमीटर है। तथा मुझे दुकान से घर तक आने जाने के लिये काफी
परेशानी होती है। मैं अपने घर से दुकान तक जाने के लिये पूर्णतया असमर्थ हूँ।
6. यह कि मेरे द्वारा दिये गये तथ्य सही व सत्य है।

2. I belong to the disabled category (category of disability). And I have a lot of trouble in commuting.
3. That all the information mentioned by me in the application form for the Chief Minister's Free Scooty Scheme to the Social Justice and Empowerment Department is completely correct and true as per my knowledge and belief. I have not hidden any fact.
4. That I have not previously taken any loan under the Chief Minister's Free Scooty Scheme and any other scheme of any government and non-government bank / institution.
5. That I run an electrical shop in village Chirana for about two years and the distance of my shop from my house is about 3 kilometers. And I have a lot of trouble in commuting from the shop to home. I am completely unable to go from my house to the shop.
6. That the facts given by me are correct and true.
